Echevarri Sports Camp Registration
1. Informed Consent and Acknowledgement
I, the undersigned parent or legal guardian of the minor child named in this registration, hereby give full approval for their participation in all activities organized by Echevarri Sports during the selected camp session.
I understand that participation in sports and physical activities involves inherent risks, including but not limited to sprains, fractures, concussions, heat-related illnesses, or more severe injuries such as paralysis or death. I knowingly and voluntarily assume all such risks, whether foreseen or unforeseen, and accept full responsibility for any resulting injury, illness, or accident.
I release and agree to hold harmless Echevarri Sports, their directors, officers, staff, coaches, volunteers, agents, and any sponsoring organizations or facilities used for camp activities, from any and all liability, claims, or demands arising out of or related to my child’s participation, including transportation to and from camp sessions.
2. Photo and Video Consent
I acknowledge and accept that photographs and video recordings may be taken during the camp for use in marketing, social media, internal documentation, or promotional materials. I hereby grant permission for the use of my child’s image, likeness, or voice without compensation.
If I prefer to opt out of this consent, I will submit a written objection via email prior to the camp’s start date.
3. Medical Release and Insurance Disclaimer
As parent or guardian, I hereby authorize the staff of Echevarri Sports, or any responsible adult at the time of an incident, to arrange for medical treatment in the event of an emergency involving my child. This includes permission for diagnosis and treatment by a licensed medical professional, as deemed necessary to prevent further harm, pain, or medical complications.
I grant permission to the attending physician to conduct minor surgical procedures, administer medications, perform x-ray examinations, or other treatments as may be necessary. I understand that every effort will be made to contact me as soon as possible in the event of a serious emergency. This release shall remain valid for the duration of the registered season or session.
I understand that while Echevarri Sports holds a valid civil liability insurance policy covering organizational responsibility, this does not extend to personal accident, health, or emergency medical costs for individual participants. Families are expected to hold their own insurance for such coverage.
4. Legal Jurisdiction
This agreement shall be governed and interpreted according to the laws of Spain, and any legal proceedings shall be brought exclusively before the courts of Marbella (Málaga), Spain.
5. Acknowledgment and Signature
By signing below (manually or electronically), I confirm that I have read, understood, and agreed to the above Terms and Conditions. This acknowledgment shall have the same effect as a handwritten signature and will remain valid throughout the duration of the registered camp program.
